2. Enrollment & Requirements

To enroll a child in our in-home daycare, parents/guardians must fulfill the following prior to the child's first day:

  • Documentation: Provide a fully completed Arizona Emergency Information and Immunization Record card (Form ADHS/CCS-106A) or a valid exemption form.

  • Childcare Agreement: Sign our formal, off-line Childcare Agreement detailing tuition rates, holiday schedules, payment deadlines, and operational rules.

  • Capacity Limits: Placement is subject to availability and strict state-mandated adult-to-child ratio limits.

 

3. Website Use & Intellectual Property

  • Permitted Use: You may view, download, and print pages or forms from this website solely for your personal, non-commercial use in evaluating our childcare services.
